Evaluation Form

The congregation is invited to offer input to the Religious Education program. The DRE Advisory and Religious Education Committee welcomes comments from ALL members and friends.

Evaluations have been mailed to all registered families, and extra evaluations are available in the social hall and the website for people who do not have children in the program.

There is a box in the social hall for completed forms or you may mail them to Hilary Gerdes (address is on form). Evaluations are due by May 7 and results will be published in the Social Hall by the end of May.

You may download the PDF of the form, complete it, and mail it to the address on the form by May 7. Thank you.
