Want to register your child or youth for the Religious Education program? If you have visited a few times and decided that this spirutual home is a good match for your family, fill out the registration form and return to the church or fax to 541-686-1260.

SUNDAY VISITOR FORM

Want to visit our Religious Education program? You can fill out a Visitor Form (at the office) for your child/youth and bring it with you your first day. (Forms are also available from your child's teacher on site as well). After you visit up to three times, you can then decide whether our program is a good match for your family at which point you would fill out a regular registration form.

Lost your field trip form? Call the office during office hours to let us know you will be using this form, fill this form out, including the title and date of the event, and bring to church during office hours.
